Reports from the Madison County Health Department, based on recent inspections of restaurants and other places that serve food. A critical violation is one that could significantly contribute to contamination, illness or environmental hazard. A non-critical violation is a minor violation, unlikely to contribute to contamination, illness or environmental hazard. Potentially hazardous food is food that needs to be kept below 31 degrees or above 135 degrees. Restaurants may have corrected violations between the time of the inspection and the publication of this report. All reports are from routine bi-annual inspections, unless otherwise indicated. * Indicates violations repeated from earlier inspection. Must correct immediately or further action will be taken. "R" Indicates repeated violations. All food and self-serve items must be stored at least six inches above the floor.

VILLAGE PANTRY, 1629 Raible Ave., Anderson — Aug. 24 — Non-critical violation: Light shield not present in the walk-in freezer.

THE CURVE INN, 114 S. Park Ave., Alexandria — Aug. 25 — Critical violations: 1) Inside of reach-in refrigeration units are not clean. 2) Grill and area around it are not clean.

Non-critical violations: 1) Grease trap is open. 2) Beer stored in ice meant for human consumption. 3 ) Floor (especially under and around equipment) is not clean. 4) Vents above grill and deep fryer have grease and dust build up.

PIZZA KING, 1409 Ind. 9, Alexandria — Aug. 25 — No violations found.

POINT TAVERN, 905 N. Lincoln, Alexandria — Aug. 25 — Non-critical violation: Thermometer not provided in freezers in kitchen.

SUBWAY, 1407 Ind. 9, Alexandria — Aug. 25 — No violations found.

KING GYRO, 2743 Nichol Ave., Anderson — Aug. 26 — Critical violations: 1) Soda nozzles (especially Coke) extremely soiled. 2) Certified food handler not provided.

Non-critical violations: 1) Approximate 1 1/2” gap found at rear exit door. 2) Thermometer not provided in “vitamin water” reach-in cooler. 3) Ice scoop handle found lying directly on ice in bin at front counter. 4) Wiping cloths not being kept in sanitizing solution between uses. 5) Floor in walk-in cooler no longer has a smooth, cleanable surface.

PENN STATION, 4231 Scatterfield Road, Anderson — Aug. 26 — Critical violations: 1) Sliced ham and turkey found not date marked. 2) Full chemical bottle found not labeled. Non-critical violations: 1) Tubs of sugar and seasonings found not labeled with common name. 2) Reach-in refrigeration unit, between grill and toasting oven, did not have a thermometer.

PIZZA HUT, 2607 Nichol Ave., Anderson — Aug. 26 — Non-critical violations: 1) Area under and behind dishwasher found soiled, with several gnats present. 2) Outside garbage dumpster lid found open.

STARBUCK’S 9355, 4235 Scatterfield Road, Anderson — Aug. 26 — Critical violations: Four gallons of milk were found in reach-in cooler not date marked. Non-critical violation: Light shield not provided over bulb in walk-in freezer.

SUBWAY, 1904 Scatterfield Road, Anderson — Aug. 26 — Critical violation: Hand sink blocked by serving utensil. Non-critical violation: Thermometer found under meats and vegetables on the prep line.

ARBY’S, 2820 Broadway, Anderson — Aug. 27 — Critical violations: 1) (R) Hand sink blocked by a large trash can. 2) Slicer very soiled. 3) (R) Sliced cheese in sandwich prep area was at 51 degrees and chicken salad at 40 degrees.

BURGER KING, 2910 Broadway, Anderson — Aug. 27 — Critical violation: Timer not set on the potentially hazardous food at the grill. — Non-critical violation: Back door was not completely closed.

K-MART, 2828 Broadway, Anderson — Aug. 27 — Critical violation: Baby formula outdated (Similac Advance Infant Formula, 6-8 oz. bottles dated 08-01-09). (Corrected immediately.)

McDONALD’S, 2830 Broadway, Anderson — Aug. 27 — Non-critical violation: Box of single service cups stored directly on floor.

APPLE BARN, 6356 W. Fall Creek Dr., Pendleton — Aug. 28 — No violations found.

KING HUN RESTAURANT, 110 S. Pendleton Ave., Pendleton — Aug. 28 — Critical violation: (*) Raw chicken and pork found stored directly above uncovered ready-to-eat foods and sauces in reach-in refrigerator. (Corrected immediately). Non-critical violations: 1) (*) Floors and walls in kitchen have severe accumulation of grease build up (especially in the wok area) (Given three days to correct.) 2) (R) Uncovered food found in reach-in refrigeration. (30 minutes to correct).

THE NILE RESTAURANT, 723 E. Eighth St., Anderson — Aug. 28 — Critical violation: Milk found in the mini refrigerator not date marked.

RAX RESTAURANT, 1106 Main St., Anderson — Aug. 28 — Critical violation: Sliced ham found being held past seven-day hold date (8-20-09 on container).

RICKER’S PETRO MART, 925 W. 38th St., Anderson - Aug. 28 — No violations found.

ANDERSON HIGH SCHOOL, 4610 Madison Ave., Anderson - Aug. 31 — Critical violations: 1) Full chemical bottle stored on a food preparation surface. 2) Pizza and mashed potatoes, in the food warmer and on the serving line, were found at 118 and 121 degrees, respectively.

COX SUPERMARKET, 302 N. Harrison St., Alexandria — Aug. 31 — No violations found.

LITTLE CAESAR'S PIZZA, 921 E. 38th St., Anderson — Aug. 31 — Critical violation: Bottle containing chemical was found not labeled. Non-critical violation: Two containers of seasonings found without labels.

THE PIT STOP, 1220 Pendleton Ave., Anderson — Aug. 31 — Critical violations: 1) Sliced ham, in refrigerator, not date marked. 2) No employee has a certified food handler permit. Non-critical violations: 1) (*) Pop dispenser gun at the bar found soiled. 2) No thermometers provided in two freezers above refrigerators. 3) Walls behind fryer and grill very soiled. 4) Back door has an approximate 1/2” gap at bottom. 5) Floor under broaster extremely soiled. 6) Ice scoop found directly on the ice.

RICK'S FOOD MART, 925 MLK Jr. Blvd., Anderson - Aug. 31 — Non-critical violations: 1) Soft drink dispenser nozzles are not clean. 2) Employee’s coat stored on food product in storage room.

TARGET, 5501 Scatterfield Road, Anderson - Aug. 31 — No violations found.

VILLAGE PANTRY 580, 803 Madison Ave., Anderson - Aug. 31 — No violations found.

BULL DOG CORNER, 701 Main St., Lapel - Sept. 10 — No violations found.

DOLLAR GENERAL, 9288, 620 W. Pendleton Ave., Lapel - Sept. 10 — No violations found.

SUBWAY 27476, 650 W. Pendleton Ave., Lapel - Sept. 10 — Non-critical violations: 1) Thermometer not provided in reach-in cooler below toaster oven. Cooler temperature found at 50.8 degrees. 2) Paper towels not provided at hand sink in rear area. 3) Single service items (lids) found stored directly under drainage line at hand sink at front counter.

A & S Petroleum, 1806 E. Ind. 38, Markleville - Sept. 11 — Critical violation: (*) Certified food handler not provided. (Manager said test has been taken, however, results have not yet been reported.) (Given 30 days to correct.)

Non-critical violations: 1) Soap and paper towels not provided at hand sink.

LOS COMPADRES, 3724 Columbus Ave., Anderson - Sept. 11 — Critical violations: 1) Raw beef and pork found stored directly above cheese in walk-in cooler. 2) Bottle containing bleach found stored directly on food preparation surface. Non-critical violations: 1) Soap not provided at hand sink in meat cutting area. 2) Dumpster lid found open. 3) Thermometer not provided in Nestle cooler chest by front counter. 4) Light shields not provided on lights in produce display case. 5) Bag of coconuts found stored directly on floor in rear storage area.

MARKLEVILLE PIZZA, 35 W. Main St., Markleville - Sept. 11 — Non-critical violation: Hand sink and three-bay sink were not sealed to the wall. (Given 30 days to correct.)

PIZZA KING 84, 604 W. Pendleton Ave., Lapel - Sept. 15 — No violations found.

RICKER'S 5, 1700 Main St., Lapel - Sept. 15 — Non-critical violations: 1) Thermometer not provided in “Good Humor” ice cream case. 2) Cooler, holding pre-packages sandwiches, running at 55.7 degrees.

ART'S PIZZA, 4762 Scatterfield Road, Anderson - Sept. 16 — Critical violation: Two bottles containing chemicals, found below three-bay sink, were not labeled.

WHITE RIVER PAINT BALL, 5211 S. New Columbus Road, Anderson - Sept. 18 — Critical violation: Several bottles of chemicals were found below three-bay sink without labels.
